Professional Referral
Before submitting a referral, please read the information below.
This ensures both your and your client understand the process and it helps our team connect with them appropriately.
Submit a Referral
If you’re a GP, clinician, or health professional referring someone to Ballarat Men’s Mental Health, please use our secure referral form:
What BMMH Does
Ballarat Men’s Mental Health is a triage and referral service. We assess the needs of the men referred to us, connect them with the right support, and where eligible, provide financial assistance for therapy with one of our contracted clinicians. We are not a counselling service and do not provide counselling directly.
Once a referral is received, our triage team will contact the participant at their scheduled call back time. Our team will make two attempts to reach your client. If contact cannot be made after two attempts, we are unfortunately unable to continue to pursue the referral.
Supporting Your Client
to Book
To help us connect with your client as quickly as possible, we ask that you support them to book their call back and complete the consent form before or at the time of submitting your referral.
The participant must be present to complete the booking as they are required to read and sign the consent form themselves. You can share the booking link below with your client or sit with them while they complete it:
Please ensure all relevant participant details are included on the referral form at the time of submission so our triage team has everything they need to make contact. If a booking has not been made, we will do our best to reach your client through our scheduled call back process, however we are not able to guarantee contact if the participant is not expecting our call.
Please Note:
Ballarat Men’s Mental Health is not an emergency service. For urgent support, contact Grampians Mental Health Services on 1300 247 647 or Lifeline on 13 11 14. If a life is in danger, call 000.
